RELEASE AND WAIVER OF CLAIMS As parent or legal guardian of the children, I hereby represent that the Child is in good health an capable of fully participating in the Chicago Red Stars Soccer Camps held by the Chicago Women's Soccer Academy, LLC d/b/a Chicago Red Stars ("Chicago Red Stars") at the above checked facility ("Camps'). Further, I understand that participation in the Camps involves risk and possible serious injury as are inherent in any athletic activity. I represent to Chicago Red Stars that Child has medical health insurance to cover any injuries sustained as a result of participation in the Camps. I authorize the Chicago Red Stars to secure emergency treatment should Child require it. I understand that I will be responsible for any and all expenses incurred in connection with such treatment. I understand that Child is not covered by any of Chicago Red Stars health policies. I further agree to indemnify and hold harmless the Chicago Red Stars and the above checked facility and their respective employees, vendors, and contractors, from any and all liability, damage, or expenses arising out of said treatment or the Child's participation at Camps. I further grant the Chicago Red Stars the right to use, for any purpose whatsoever without compensation to me, player's name, picture and/or likeness in printed, broadcast and other material concerning the Camps provided such use is related to player's status as a participant in the Camps. All negative and positives, together with print, shall constitute Chicago Red Stars' property, solely and completely.
CANCELLATION POLICIES If a cancellation should ensure, CRSA will call all relevant campers as early as possible. Otherwise, a client should assume that everything is as scheduled. Any session that is moved to an alternative location (e.g., indoors) by CRSA is treated the same as its originally scheduled session with respect to any resulting charges. A session that is cancelled by the CRA and/or RSSS after half-way through its du ration is deemed complete, and the client will be charged in full. For example, a training session or program may be cancelled partway through due to unexpected weather condition.
